I _______________________________________________________, hereby do submit to Underground Records the included pre-produced materials, in acceptable format, with this release form, for inclusion on Underground Records not-for-sale productions, lists and offerings (herefter known as productions).
I am the registered or recognized copyright holder for these recordings and do hereby allow Underground Records to use these recordings for the purpose of including them on productions. For any material I do not own the copyrights to, I realize I am fully responsible for all reporting of such use to the copyright holder, and hereby release Underground Records from any responsibility concerning mechanical licensing or performance fees and should such become due to any rights holder other than myself, I recognize and agree Underground Records is not responsible for such or similar fees.
It is recognized Underground Records will not sell these recordings outside of using them to promote my music, talent and image. I hereby grant Underground Records the right to use these recordings in the production, distribution, broadcast and performance of forms of free media. Underground Records shall not cause use of my materials in their productions to lead to commercial reproduction of said materials unless I so warrant so beforehand.
The copyright holder(s) of the materials and mechanical reproductions of the recorded music forwarded to Underground Records for included use in productions is recognized as legally being held in the name(s) residing or registered at the address(es) listed below (please give all names and separate addresses below, attach extra sheet if necessary):

License to copy;
It is hereby consented that any publisher fees, agency fees, or other fees or royalties resulting from the use of these recordings, other than the above noted rates, are my personal responsibility and does not affect the right of Underground Records to use these materials, and I hereby sanction the non-profit use of my copyrighted materials for marketing and promotions.
Underground Records shall not mass produce singular, or full program copies of the exact programs, compositions, or edits thereof regarding these compositions, or copyrighted material, outside of the inclusion on a production, unless agreement of all parties involved takes precedence outside the terms of this agreement.
Underground Records has the right to use, produce, and publish pictures, or likeliness of artist, band or label in regards to usage of these materials in conjunction with any Underground Records freely distributed production.
I hereby release Underground Records from liability stemming from usage of my copyrights except those terms explicitly directed herein. Any third party usage of these copyrights does not pertain to this agreement, and is not the responsibility of Underground Records. I hereby realize Underground Records, its agents or successors, may and should be notified of any improper use of copyright materials, and do so realize Underground Records, its agents and successors, do not intend to do harm or improperly use my copyrighted materials. For broadcast usage of these copyrights over any radio, or other audio media, other than produced mechanical reproductions used and distributed as described herein, I hereby recognize it is solely my responsiblity as to any collection of fees, permits or royalties produced or coming due thereof.
